San Diego City Attorney, July 9, 2026What happened
The story, from the record.
On July 9, 2026 the San Diego City Attorney announced a $5.25 million final judgment and four-year permanent injunction against Molina Healthcare of California over inaccurate provider directories, ending a suit the city filed in 2021 under California's Unfair Competition and False Advertising laws.
The complaint drew on Molina's own survey data. It alleged an error rate of 57.88% in the December 2018 directory and 83.32% for psychiatrists, more than 20% of surveyed providers listed but out of network, and 325 physicians flagged in 2018 still listed a year later.
The injunction requires Molina to purge inactive, unlicensed, deceased and out-of-network providers using national practitioner and death records, validate the directory to at least 90% accuracy, show when each entry was last updated, and fix flagged errors within 60 days.
None of those terms finish. Providers retire, move and leave networks every month, so a directory is only as accurate as its last check. Four years of the injunction is four years of checking every listing.
How OneShot would run it
The goal this leaves open.
OneShot was not involved in this event. This is how the goal it describes would run as one OneShot goal: what one case is, when the goal is done, the steps per case, and the known tool costs for one pass.
Keep every listing in the directory true, and show at least 90% accuracy every cycle for the four years of the injunction.
- One case
- One directory entry: a provider, at a location, in a network.
- Starts
- On a 90-day cycle for every entry, and within a day of any error a member reports.
- Done when
- Every entry is checked this cycle and ends confirmed, corrected or removed; every flagged error closes within 60 days; measured accuracy is at least 90%.
- Keeps trying
- Two calls to the office on different days, then an email, then the entry is marked unverified and sent for removal review.
- Asks a person before
- Removing a provider from the directory, and any accuracy figure reported to the court.
