ByHeart recalled two lots of its Whole Nutrition infant formula on November 8, 2025, after infants who had been fed it were diagnosed with botulism, and extended the recall to every product three days later. CDC's final count was 28 confirmed and 20 probable cases across 17 states; 48 infants were hospitalized and none died.
The recall notice went out. The product did not come off the shelves. FDA investigators made more than 4,000 retail checks and found recalled formula still for sale at more than 175 locations in 36 states, some more than three weeks after the recall. One Target store sold a can at a two-dollar discount after the recall had been expanded.
On December 12 FDA sent warning letters to Walmart, Target, Kroger and Albertsons, each with 15 working days to answer. Three days later it wrote to the whole food industry asking for written recall plans and for effectiveness checks: evidence, not assumptions, that a recall reached every consignee.
That is the shape of a goal that never finishes. A recall is announced once; the work is a check of every store, every distributor and every buyer, repeated until the count comes back clean.
